Fertility is the natural capability to produce offspring. For women, fertility is closely linked to the health and functionality of the reproductive system. Over a woman’s lifetime, her fertility undergoes significant changes, influenced by age, health, and various external factors. This article explores the different stages of a woman’s life to understand when and why fertility declines, the biological processes involved, and potential interventions to address fertility issues.

The Basics of Female Fertility

Anatomy and Physiology of Female Reproductive System

A woman’s reproductive system includes the ovaries, fallopian tubes, uterus, and vagina. The ovaries produce eggs (ova) and hormones such as estrogen and progesterone. The fallopian tubes transport the eggs from the ovaries to the uterus. Fertilization typically occurs in the fallopian tubes. If fertilization happens, the embryo implants in the uterus, leading to pregnancy.

Menstrual Cycle

The menstrual cycle is a series of hormonal changes that prepare the female body for pregnancy. It typically lasts around 28 days but can vary. The cycle has several phases:

Menstrual Phase: Shedding of the uterine lining, resulting in menstrual bleeding.

Follicular Phase: The pituitary gland releases follicle-stimulating hormone (FSH), stimulating the ovaries to produce follicles. Each follicle contains an egg.

Ovulation: The surge in luteinizing hormone (LH) triggers the release of an egg from the ovary.

Luteal Phase: The empty follicle transforms into the corpus luteum, which secretes progesterone to thicken the uterine lining for potential implantation.

Peak Fertility Period

Women are most fertile during their 20s. During this period, the ovarian reserve (the number of eggs in the ovaries) is at its peak, and the quality of the eggs is optimal. Ovulation is regular, and the chances of conceiving are highest.

The Decline of Female Fertility

Age and Fertility

Age is the most significant factor affecting female fertility. As women age, the number and quality of their eggs decline. This decline begins subtly in the late 20s and becomes more pronounced in the mid-30s. By the age of 40, fertility has significantly decreased, and the chances of natural conception are much lower.

Ovarian Reserve

Women are born with all the eggs they will ever have, approximately one to two million. By puberty, this number decreases to about 300,000. Each menstrual cycle, about 1,000 eggs are lost. Over time, the ovarian reserve diminishes, leading to reduced fertility.
